An all-inclusive approach to health care. With one monthly price for self, couple or family. This allows you to obtain wellness/sick visits with access to a provider, via, text, phone, email or virtual visit based on specific health needs.

Includes;

Provider assessment with appropriate treatment
Orders for appropriate labs and or diagnostic testing based on diagnosis. (Orders sent to the lab, diagnostic site of your choice and separately billed by this entity)
Referrals to specialists based on needs, wellness screenings

Self Price $60/mo

Self + Spouse $100/mo

Family $150/mo

Receive a 10% discount if paid in full for 6 months or 1 year.

Non-concierge basic consultation/appointment $30/visit.
